There is no routine screening for thyroid conditions by the NHS but many people have an underactive thyroid but do not know it because they have never been tested. Only when diagnosed do they realise that there was a reason for their symptoms. An abnormal result where the TSH level is high, above 4 suggests that the thyroid is underactive. You can choose to have measurements of the active thyroid hormones too, T4 only or T4 and T3 making this a very comprehensive test for thyroid function. Free T4 and Free T3 will be low when the thyroid is underactive.
A normal TSH result is very reassuring. It is unusual for TSH to be normal when the thyroid is underactive but this can happen rarely. Measuring free T4 and free T3 means that when the TSH is normal a thyroid problem can still be detected.
